Welcome to our area for new employees of the Trust.

Occupational Health questionnaire

Before you start working for the Trust, we ask you to complete our Occupational Health questionnaire.

The purpose of the questionnaire is to see whether you have any health problems that could affect your ability to undertake the duties of the post you have been offered or put you at any risk in the workplace. We may recommend adjustments or assistance as a result of this assessment, to enable you to do your job.

Our aim is to promote and maintain the health of all people at work. Before health clearance is given for your employment, you may be contacted by the Occupational Health team and may need to be seen by an occupational health advisor or physician.

How to complete your Occupational Health questionnaire

We ask that all forms are returned to us by email as follows:

• First, please download the questionnaire (word doc - 250 KB) and save it somewhere safe on your computer.
• Complete the questionnaire, making sure you save the document once you have finished.
• Please email your completed form to occupationalhealth@sabp.nhs.uk.
