Communicating your needs

The communications resources listed below are deisgned to help people with a learning disability communicate with clinical staff. They can be downloaded from www.surreyhealthaction.org

  • Easy Read Health Check Appointment Letters: a tool to help you create Easy Read health check appointment letter. You can use this if you are a GP Surgery, carer or supporter of a person with a learning disability
  • Hospital Communication Book: an easy-to-read guide with images and symbols to help people with a learning disability communicate how they feel.
  • Hospital Passports: a guide to the individuals health and wellbeing to complete in advance of the hospital visit so that hospital staff have an accurate record of key information relating to their health and medical history
  • Hospital Questionnaires: To provide feedback on the hospital stay to help staff improve the hospital experience for people with a learning disability
