Service Description

Our Community Mental Health and Recovery Service (CMHRS) provides an integrated health and social care service for clients aged 16-65.

The team focuses specifically on adults with severe and enduring mental health issues. It is a multidisciplinary team based in the community and working closely with other care providers and agencies to provide a cohesive package of care.

Carers of people who use this service are supported by two Carer Support Workers, Claire Bampton and Karina Busikool. Carers information can be found at www.sabp.nhs.uk/advice/carers.

Referral Process

We accept referrals in writing from other agencies and healthcare providers. All referrals will be screened by the duty officer and, if routine they will be discussed and allocated for assessment at the allocation team meeting, held weekly on Tuesday mornings. If referrals are more urgent the duty officer will attempt to make contact with the service user in order to assess further risk.

Referral Criteria

Adults aged 18 to 65 Suffering from acute or severe and enduring mental health problems including personality disorder.
