Service Description
Willow ward provides End of Life care for people with Continuing Care needs associated with their dementia. It follows the Gold Standard Framework and Liverpool pathway. It is a 15-bedded service, which is commissioned by local Primary Care Trust (PCT). Referrals to Willow Ward are made through Older Adult psychiatric consultants, Community Mental Health Team's and Mental Health Treatment and Assessment units - All referrals to the Willows must have been assessed by the PCT Continuing Care Nurse and have been assessed as meeting continuing care needs.
