Our Intensive Support Service for People with a Learning Disability works in partnership with existing community and liaison services to deliver 24/7 personalised care to people with learning disabilities. The aim is to create a streamlined health service which supports people in their own home, reduces the number of inpatient admissions, and provides proactive support post discharge.
