Service Description
HOMEFirst is an approach to supporting people who are at risk of repeat admission to hospital due to mental health relapse.
Developed with community partners, HOMEFirst support is available to adults aged 18 years-old and over on referral from SABP teams and partner organisations. The approach brings together multi-agency working and good quality care planning to provide enhanced community support.
HOMEFirst can draw upon additional support from Recovery and Connect workers, a Lead Occupational Therapist and Carer, and Lived Experience workers, to help people manage their mental health and wellbeing at home.
Referral Criteria
- A recent history of repeat hospital admissions owing to complex and chronic psychotic illness or mood disorder (primary).
- Receiving support from several agencies.
- Accommodation problems.
- Experiencing Isolation/loneliness.
- Difficulty engaging in meaningful activity and managing activities of daily living.
- Difficulty taking medication.
