2 September 2026

We are changing the way we organise our community mental health services to provide more joined-up care and make it easier for people to get the right support.

We plan to introduce 13 new Neighbourhood Mental Health Teams from the beginning of November 2026, subject to final planning.

What is changing?

We are bringing together our adult, older adult and primary care mental health teams into 13 Neighbourhood Mental Health Teams.

By bringing staff together in this way, we aim to provide more joined-up care and make it easier for people to access the support they need.

What does this mean for me?

For many people, there will be little or no change.

If you currently receive care and support from one of the affected services:

  • you will continue to receive care from us
  • your appointments and treatment will continue as planned
  • you should continue to attend your appointments as usual.

For some people, the team or Lead Professional supporting them may change.

If this affects you, we will contact you directly. We will explain what is changing and give you an opportunity to ask questions.

What happens if my Lead Professional changes?

We will plan your handover carefully so that your care can continue without interruption.

Your current Lead Professional will work with the professional taking over your care to share the information they need to support you.

Do I need to do anything?

No. You do not need to do anything at the moment.

Please continue to attend your appointments as usual.

We will contact you directly if the changes affect you or there is anything you need to do.

When will the changes happen?

We are currently planning to introduce the new Neighbourhood Mental Health Teams from the beginning of November 2026. This is subject to final planning.

We will update this page as our plans develop.

I have a question or concern

If you have questions about what these changes could mean for your care, please contact your usual team.

They will be able to talk to you about your care and any changes that may affect you.
