Service Description
Jasmine House at Primrose is a respite care unit and can provide a service for up to eight adults with a learning disability and health care needs. All facilities are on the ground floor.
Jasmine House is based at Primrose Ward at The Meadows in Epsom. To find us, use the map on the right.
Referral Process
We accept referrals from families, Care Managers & GPs. All referrals must be sent to the Community Team for People with Learning Disabilities
Referral Criteria
Adults aged 18 years and above with a learning disability, living in Surrey with health care needs
