Service Description
We offer home visits to people in South West Surrey who are aged between 18 to 65 years and suffering from a mental health crisis. The service will aim to support young people who are 17 years as part of their transitioning care plan from the CAMHS service.
Our medical teams assess whether people require hospital admission or whether they can be treated at home, offering alternative treatment to avoid admissions where possible.
If hospitalisation is necessary, we stay actively involved in planning the return home and provide intensive care following discharge (or during periods of leave).
We regularly visit the individual until the crisis has been resolved and ongoing care is in place, operating 24 hrs a day, 365 days a year.
