Community Mental Health Recovery Services (CMHRS)

Our community teams are dedicated to helping people with severe or enduring mental health problems back on the road to recovery.

Our teams of health professionals include people from different disciplines including Doctors, Nurses, Psychologists, Occupational Therapists and Social Workers.

We provide evidence based treatment for a range of mental health challenges and can refer to a host of different community organisations both in the statutory, independent and voluntary sector to access additional support. We are open Monday to Friday, from 9am to 5pm. If you would like to be referred to this service, please contact your GP who will assess your suitability and refer you to us if it is appropriate.

If you receive care from one of our Community Mental Health Teams and would like to move to a different team, you can ask us to review your care arrangements.

We will consider your request and discuss whether moving to another team would be right for you.

How to request a change

Contact the service manager of your current team and explain that you would like to receive your care from a different team.

What happens next?

Your care team will review your request with your responsible clinician and other healthcare professionals involved in your care.

They will consider your clinical needs and whether moving to another team would be appropriate.

If a move is agreed

Your current team and the team you would like to move to will discuss the arrangements. The team managers will organise the transfer and let you know what happens next.

If a move is not agreed

Your care team will explain the reasons for the decision and discuss how they can best meet your needs within your current team.

How long will it take?

The process can take up to 28 days, depending on your circumstances.

You will continue to receive care from your current team while your request is being reviewed. Your team will keep you updated on progress.

What if I'm unhappy with the decision?

If you're unhappy with the decision, you can raise your concerns with your service manager.

They will review your concerns and try to resolve them.

If you're still unhappy, you can contact our Patient Advice and Liaison Service (PALS), which can help you raise your concerns through the Trust's complaints process.
