Young Onset Dementia is a small Team which works across each Community Mental Health Team for Older People in Surrey & Borders Partnership NHS Foundation Trust. We offer support and guidance to those with Young Onset Dementia and their carers who have been referred to us by their GP. We offer one to one support throughout and after your diagnosis.
