Service Description
We are a community team supporting people who have learning disabilities and their families in the Redhill, Reigate, Banstead and Horley area.
We work with people who have an impairment of social functioning, linked to an IQ of 70 or below, who may have difficulty managing their health needs independently.
We are a multi-professional team which includes: community nurses, psychiatrists, psychologists and a range of therapists. We undertake specialist secondary health assessments and therapeutic interventions. We also provide advice and support to enable people with learning disabilities, who often have complex and continuing health needs, to maintain and improve their health wherever possible.
We support people to access primary and secondary health services, where facilitation by a specialist learning disability practitioner is required. The team can also help people with a learning disability with a range of other specialist services such as health promotion and the development of healthy lifestyles.
We work in partnership with other mainstream health services and appropriate specialist services as well as a wide range of other statutory and non-statutory agencies.

Referral Criteria
The CTPLD will provide support to adults over the age of 18 who have a learning disability, where their primary need is for a Specialist Learning Disability service. The service will work in conjunction with other stakeholders to offer support and treat as appropriate.
To be eligible for services the following criteria apply:
- An IQ of below 70 and a measured impairment of social functioning.
- An assessment of social functioning and communication skills should also be taken into account when determining needs.
- The definition covers adults with autism who also have learning disabilities, but not those with a higher-level autistic spectrum disorder who may be of average or even above-average intelligence.
- People referred to the team must also have specialist healthcare need that cannot be met through mainstream services.
- To support the transition from child to adult services the team are also involved in the assessment and planning for children with learning disabilities post 14.
- People must have been registered for a period of no less than twelve weeks with a G.P. practice within the geographical boundaries of the team.
Exclusions:
- People who have a diagnosis of an acquired brain injury that occurred after the age of 15 years.
- People with serious and chronic mental illnesses such as schizophrenia whose pre-morbid IQ was in the normal intelligence range.
- People who (following a period of assessment and/or treatment) are deemed to be inappropriately placed and for whom no arrangements are in place to seek a more appropriate placement.
- The presence of a low intelligence quotient, for example an IQ below 70, is not, of itself, a sufficient reason for deciding whether an individual should be provided with additional health and social care support.
Waiting Times
Initial assessment will take place within 28 days of referral
