How to request a consultation
Professionals can email HealthPsychologySW@sabp.nhs.uk to obtain an initial consultation request form. Please complete and return this via email and we will review your request in our request meeting.
Community Health Psychology is a consultation service and cannot offer crisis assessment, urgent clinical intervention, or direct risk management.
Responsibility for ongoing care and risk remains with the referring professional or team.

Inclusion criteria:
- The referring professional to be available for consultation.
- The client has a GP in the Guildford & Waverley location.
- We are an adult service (over 18 years old).
- The client with remain under the care of the professional/team seeking consultation with Community Health Psychology.
- We do not offer psychological therapies directly, however, we can discuss signposting options with you should this be required.
Community Mental Health Recovery Service: Elmbridge
Our Community Mental Health Recovery Service CMHRSs are integrated health and social care teams with a single team manager. The teams focus specifically on adults with severe and enduring mental health issues.

Referral Process
We accept referrals from: GPs, IAPT talking therapy providers and social services.
- GPs can refer to us using the NHS Electronic Referral Service
- Other referrers can email a referral to: spa@sabp.nhs.uk
- Call 0300 456 8342 to speak to our Single Point of Access for Adult Mental Health Services / Mental Health Crisis Helpline if a same day response is needed or you would like to follow up on a referral. We are open 24 hours a day, seven days a week
Waiting Times
28 days for routine appointments, five days for urgent appointments and emergency referrals will be assessed on the same day.
Referral Criteria
Adults aged 18-65 who are experiencing acute or severe and enduring mental health problems, including personality disorder, that requires assessment and intervention from a secondary care team.
