Service Description
Brook House is a long-term, independent-living service for people with learning disabilities. The four flats are located in Ottershaw, Chertsey.
Registered with the Care Quality Commission as a care home facility with nursing, the flats are run by an established team qualified in supporting people with learning disabilities, mental health needs and behaviour that can challenge. Each individual is allocated a key worker, and the management structure includes a nurse who supports the team in developing the service, and provides specialist input around people’s physical and mental health.

Referral Criteria
Referrals are welcomed from people hoping to use the service, as well as their advocates, family members or care practitioners. Brook House is suitable for males or females aged 18 and over and physical disabilities are catered for.
