Completing your DIALOG questionnaire and viewing your DIALOG history online
We're now sending DIALOG questionnaire links to some people by text message (SMS), making it easier for you to complete them and view your DIALOG history online.
This is being introduced gradually across our services. Your healthcare team will let you know when it becomes available for you.
The information below will help you open and complete your DIALOG questionnaire and access your DIALOG history.

It’s your choice
- Completing the questionnaire is optional. If you choose not to complete it, just let your healthcare professional know at your next appointment.
- If you’d prefer not to receive future DIALOG questionnaires by text message, simply let your healthcare professional know.
- If you cannot complete the DIALOG questionnaire online or you would prefer to complete it a different way (e.g. with your clinician), then let your healthcare professional know at your next appointment.

You'll receive a link to open the questionnaire by text message.

You’ll need to enter your date of birth to open the questionnaire.

Each link is unique and you can only use it once.

Your healthcare professional will send you a new link each time they ask to you to complete a questionnaire.

The link expires after 28 days. If your link has expired, ask your healthcare professional for a new link at your next appointment.

You don’t have to answer every question, but you must respond to at least one to submit the form.

There are no wrong answers. Just choose the option that best describes how you feel today.

If you’d like to explain your answers in more detail, you can do this at your next appointment with your healthcare professional.

The questionnaire has 11 questions. It cannot be saved as a draft, so please complete it in one go.

You can save or print your answers if you want, but you do not need to as your healthcare professional will automatically have access to them through your patient record.

Once you've submitted the questionnaire, you can close your browser. There is no need to log out.

Your healthcare professional will discuss your responses with you at your next appointment.
The My Care patient portal is a secure and easy way to view your DIALOG history. It includes all DIALOG questionnaires you've completed, whether online or on paper.

Go to https://mycare.sabp.nhs.uk/welcome to access the portal using your NHS login. This is the login you use to access the NHS app. If you don’t have an NHS app login then you can register here.

The first time you log in, you’ll be asked to give permission for My Care to use some of the details linked to your NHS login. These include your first and last name, date of birth and email address.

At the moment, My Care lets you view your DIALOG history. You may be able to do more through the portal in future.

It is only for viewing questionnaires you have already completed. You cannot complete questionnaires through the portal.

There’s no need to print your DIALOG history for appointments as your healthcare professional already has access to it.
No. Surrey and Borders’ My Care patient portal, which enables you to view your DIALOG history, is separate from the MyCare patient portal that allows patients at Royal Surrey County and Ashford and St Peter’s Hospitals to view their appointments and appointment letters.
