At Surrey and Borders Partnership we are dedicated to delivering NHS England’s Long Term Plan, an inspiring national strategy for transforming community mental health services across the entire country.
In 2019, our Trust was identified as one of 12 early implementers of the NHSE Long Term Plan. We received an aditional £11 million to modernise our services, which have remained virtually unchanged for the past two decades. We are delighted to be able to bring local services such as GP practices and specialist community mental health services, known as primary care and secondary care services, closer together through this extra investment.
Accessibility is key, and our aim is to make it easier for people to access the services they require in their local communities. By introducing new services to GP Practices and adopting innovative new ways of working in the delivery of community mental health services, we can extend our reach and provide thousands more adults living with significant mental health disorders with the specialist help they need, when they need it.

We visualise a future where individuals, from young people over 18, to adults and older people, embrace life to the full, reassured they have access to a wide range of specialist mental health care services in the heart of their local communities should they need help
Our mission is to break down barriers to care, ensuring swift and equal access to services through GPs and dedicated mental health service teams.

• Fewer Repeat Referrals: No more repeat visitings to different professionals seeking help and fewer repeat referrals to different services for help
• Localised Support: Increased access to mental health support in your local community. We are committed to bringing mental health services closer to you.
• Personalised Care: Sharing your story with multiple professionals can be exhausting. Our approach, in collaboration with a broad range of community organisations, fosters real-time information sharing. That means less need to retell your stories and more opportunity to tackle the outside pressures impacting on mental health and wellbeing.
Our services will be delivered in a true partnership with organisations in the community allowing ‘whole-person’ care planning. Issues related to housing, social care, debt, rent arears, addiction and more, can be addressed across partners reducing the stresses and strains in someone’s every day life to further aid their recovery.
