Community Teams for People with Learning Disabilities

Our Community Teams for People with Learning Disabilities provide specialist health assessments, interventions, and treatments for adults with learning disabilities who may struggle to access mainstream healthcare services. These multidisciplinary teams work closely with each person to promote better health and wellbeing, reduce health inequalities, and make sure their care is tailored to their needs.

Intensive Support Service

The Intensive Support Service offers urgent care for people experiencing a crisis or are in need of additional help. The team works quickly to provide support and stabilise situations, aiming to prevent hospital admissions.

Liaison Nurses

Liaison Nurses work with people with learning disabilities when they need hospital or community healthcare. They help to ensure reasonable adjustments are made and that people receive high-quality, tailored care during their medical appointments or treatments.

MyTime Day Centres

MyTime day centres provide a safe and engaging space where people with learning disabilities can build skills, make friends, and enjoy a range of activities. From arts and crafts to cooking and fitness, there’s something for everyone.

Short Break Services

Our Short Break Services provide respite care for families and carers, offering people with learning disabilities a comfortable and welcoming environment for temporary stays. These breaks help carers recharge while ensuring continuity of care for the person they support.
