HOMEFirst

HOMEFirst is an approach to supporting people who are at risk of repeat admission to hospital due to mental health relapse.

Developed with community partners, HOMEFirst support is available to adults 18 years and over on referral from SABP teams and partners organisations. The approach brings together multi-agency working and good quality care planning to provide enhanced community support.

HOMEFirst can draw upon additional support from Recovery and Connect workers, a Lead Occupational Therapist and Carer, and Lived Experience workers, to help people manage their mental health and wellbeing at home.

i-access Drug & Alcohol Service

i-access supports people in Surrey who want help with their problematic drug use and / or are highly dependent on alcohol and want to stop drinking. People can refer themselves or we accept referrals from GPs, social services, Community Mental Health Recovery Services (CMHRS), non-statutory drug & alcohol services, hospitals, other mental health professionals and probation services. Our main office is in Guildford but we run clinics across the county.
