Our Community Mental Health Teams for Older People (CMHTOP) provide assessment, diagnosis, treatment and support for people over 65 who have mental health difficulties, changes in memory or thinking, or dementia.

Our teams support people with conditions such as depression, anxiety, bipolar disorder and psychosis, as well as people experiencing memory problems.

How we can help

Our teams include doctors, mental health nurses, occupational therapists, psychologists, social care practitioners and support workers.

We work with other health and social care services and voluntary organisations, including Alzheimer's Society, to support people and their carers.

The care and support offered will depend on each person's needs. We will discuss the available options with them.

How to access our services

Referrals are made through GPs and healthcare professionals.

Our team reviews referrals and decides which assessments, treatments or support may be most appropriate.

Intensive Support Team for Dementia

Our intensive support team works alongside our community services by providing additional assessments and interventions for people with advanced dementia and very challenging behaviour. We provide home-based assessment and support to families and other professionals involved in a person's care over a period of up to 12 weeks. More information
