Developed with community partners, HOMEFirst support is available to adults 18 years and over on referral from SABP teams and partners organisations. The approach brings together multi-agency working and good quality care planning to provide enhanced community support. HOMEFirst can draw upon additional support from Recovery and Connect workers, a Lead Occupational Therapist and Carer, and Lived Experience workers, to help people manage their mental health and wellbeing at home.
