HOMEFirst
HOMEFirst is an approach to supporting people who are at risk of repeat admission to hospital due to mental health relapse. Developed with community partners, HOMEFirst support is available to adults 18 years and over on referral from SABP teams and partners organisations. The approach brings together multi-agency working and good quality care planning to provide enhanced community support. HOMEFirst can draw upon additional support from Recovery and Connect workers, a Lead Occupational Therapist and Carer, and Lived Experience workers, to help people manage their mental health and wellbeing at home.

Working with the Mental Health Integrated Community Service team, your GP is now able to offer this specialist mental health advice and support service to people over 18 years of age with significant mental health concerns, including people over the age of 65.
