Dementia and your environment
People with young onset dementia may have difficulties making sense of the environment around them. Our tips for people with dementia and their carers to navigate these changes

HOMEFirst
HOMEFirst is an approach to supporting people who are at risk of repeat admission to hospital due to mental health relapse. Developed with community partners, HOMEFirst support is available to adults 18 years and over on referral from SABP teams and partners organisations. The approach brings together multi-agency working and good quality care planning to provide enhanced community support. HOMEFirst can draw upon additional support from Recovery and Connect workers, a Lead Occupational Therapist and Carer, and Lived Experience workers, to help people manage their mental health and wellbeing at home.
