Community Hubs: bringing your local services together
We have gradually brought our community services for people of all ages together into improved accommodation across Surrey and north east Hampshire. This affects the majority of our community learning disability and mental health services for people of all ages. Our inpatient/residential and day services are not relocating to our Community Hubs.
People still receive support from the same person but may be asked to attend a different location for their appointment.
We have maintained a widespread presence across the county so that people won’t have to travel far and, just as we do now, we will continue to see people at home or at other agreed venues, such as a GP practice, if that’s more convenient.
We’ve done this to:
- Create a more modern, welcoming environment and replace buildings which are no longer fit for purpose due to their poor condition, layout or facilities
- Make us more accessible by public transport and for those with additional needs
- Improve people’s experience of our services
- Encourage better communications and more joined-up working by locating teams together
- Make our services more affordable – and sustainable - by making better use of the space we have.
